Heritage Hall Grundy is located in the scenic Appalachian area of southwest Virginia where West Virginia, Kentucky and Virginia all come together. Our award-winning facility is the only skilled nursing and rehab center in Buchanan County and we pride ourselves in taking a multidisciplinary approach to address the physical, emotional, social and spiritual needs of each of our residents.
